Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) work with your doctor(s) to provide home care services. Their assignment, establishment of nursing care plan, skilled treatment and support provide the highest quality care you need to recover at home. A registered nurse is available for admissions, urgent situations or questions 24 hours a day.

Home Health Care Nurses are able to provide:

  1.  Skilled nursing assessments
  2. Wound Care and dressing changes
  3. Injections
  4. Catheter care
  5. Ostomy care
  6. Intravenous fluid/medication management
  7. Diabetes Mellitus Care
  8. Vital Signs Monitoring
  9. Teaching to patient and family about safety, medications, diet/nutrition and disease.

 

  

Certified Home Health Aides

Home Health Aides are Certified Nursing Assistants who provide assistance with personal care:

1. Eating

2. Bathing

3. Personal Hygiene

4. Ambulation

 

  

Medical Social Services

Our Medical Social Services staff helps patients and family members understand and cop with problems caused by illness injury.

  1. Assessment of emotional      and social factors related to the patient and family
  2. Evaluate the home and      family environment
  3. Counseling and education      to maximize strengths and coping

Physical Therapy

Our physical therapy staff will come to your home to assess and treat functional impairments that result from injury, stroke, surgery, fractures, or other medical conditions. Physical therapy services include:

  1. Therapeutic exercises
  2. Gait training
  3. Instruction in using      walked and cane
  4. Pain management
  5. Education on safety      precautions

 

  

Occupational Therapy

The Occupational Therapist is a qualified healthcare professional who directs, supervises, evaluates and provides occupational therapy services to clients in the home by providing:

  1. Rehabilitative care for      patients needing assistance
  2. Training in activities of      daily living

 

  

Speech Therapy

  1. Help patients to improve      speech/communications
  2. Evaluate and treat      swallowing disorders
